At a recent Support at Home Provider Forum, providers identified an opportunity to improve communication and support safer, more timely hospital discharges for older people across the Illawarra Shoalhaven region. This idea has developed into the Orange Folder initiative.
The Orange Folder is a proposed resource kept in an older person’s home that can travel with them if they are admitted to hospital. It aims to provide hospital staff with a clear snapshot of the person’s care supports, home environment, mobility, equipment, key contacts, and other important information to support better decision-making and discharge planning.
This workshop will focus on co-designing the Orange Folder to ensure it is practical, easy to use, and valuable for both providers and hospital teams. Participants will have the opportunity to review the draft resource and provide feedback on what information should be included, refined, or changed.

Organisations involved in supporting older people through care transitions and discharge planning
Together, we aim to develop a practical resource that improves outcomes for older people while remaining achievable for the sector to implement.
